Crozer-Chester Medical Center and Taylor Hospital closed in 2025. This is an independent guide, not affiliated with Crozer Health, Prospect Medical or any hospital.
Crozer medical records
Crozer Health closed in 2025, and Morgan Records Management now holds its patient records. You can request them online, and the fee is covered for most former patients. If you were looking for the old Crozer patient portal, here's what changed and where to go instead. However, records nobody asks for by March 2, 2027 will be destroyed.
01 — Steps
These steps follow Delaware County's May 2026 guide. However, the form may change, so follow what Morgan's site shows you.
First, go to crozer.morganrecordsmanagement.com and click Request My Records. Note that you can only request online.
Next, choose General Medical, Radiology, Maternity, Endoscopy, Oncology, Behavioral Health, Billing, Pathology report or Pathology specimen. Because each department is a separate request, repeat these steps for every kind of record you need.
Then select Pennsylvania and the matching Crozer option, for example “Crozer – General Medical Records.”
After that, enter your name, date of birth, phone and email, plus the reason for the request.
Next, verify your identity with your Social Security number or a photo of a government-issued ID.
The county guide lists secure electronic delivery at $0. However, a USB drive sent by mail may carry a fee. Finally, e-sign the authorization and submit.
Write it down, because Morgan's site has a status page where you can check your request with that number.
02 — Cost
In August 2025, a court approved a deal in which the Foundation for Delaware County pays $3 million toward record-request fees. That covers about 43,000 former patients. Before then, Morgan charged $35 for an emailed copy and $75 for a USB drive.
Paid before the deal? Delaware County says you can ask for a refund through the Morgan site.
Choosing a mailed USB? We couldn't confirm that the fund covers it. So check the total on the form before you pay, or pick electronic delivery instead.
03 — What's covered
Use the Radiology department, because it covers image files and digital copies of old films. However, ChristianaCare has a seven-year imaging arrangement with Morgan for former Crozer patients it now treats, and the March 2027 destruction doesn't include that imaging.
Morgan's page says “Crozer Health System and its network” but doesn't name hospitals. Since Taylor Hospital was part of Crozer Health at closing, Morgan likely holds its records too. However, we couldn't confirm records from Springfield or Delaware County Memorial, which closed in 2022.
Neither Morgan nor the county explains this process yet. Usually, though, the estate's executor or administrator requests the records and shows court papers. We'll add the exact steps once Morgan or the county posts them.
According to Delaware County, unclaimed records will then be destroyed. Afterward, we'll update this page and stop sending people to requests nobody can fill.
04 — Questions
No. Crozer Health no longer operates, and this is an independent guide. In fact, the only place to request records is Morgan Records Management's site.
No. We don't collect personal or medical information for record requests. Instead, use the Morgan site directly.
Morgan doesn't publish a turnaround time. Instead, it says it processes requests in the order it receives them. So if you need records for an appointment or a claim, request them now.
Yes. Imaging falls under the Radiology department, which is separate from General Medical. Therefore, make one request per department.
We couldn't find a published phone number or mailing address for Crozer requests. So for now, Morgan's request site is the official channel. Meanwhile, if you need care today, see open hospitals in Delaware County.
